Participating in the Patient Portal for the First Time

There are two choices for signing up for the patient portal first time:

Registration online

We suggest enrolling online when you can, as it will require fewer steps to follow.

Step 1

Fill out the online consent form online. Start by clicking the link below.

  • There will be an invitation email to access the patient portal once the forms are processed. It is your responsibility to accept this invitation. The email typically arrives to you within 48 business hours.
  • The email link expires within 90 days. Make sure to check your email in order to establish your account.
  • You will be required to reset your password each time you first sign in to the portal.

Step 2

  • The patient portal will send you an invitation email to join the patient portal after the forms are processed. The invitation must be accepted.
  • The email link expires within 90 days. Check your email to allow access.
  • You will be required to reset your password each time you first login onto the portal.
  • The security question will ask for your child's birth date and your security question.
  • You'll then create an account with your details.

Enrolling with physical paperwork

Step 1

You will need to fill out the Authorization for Participation on the MyChildrensMercy Patients Portal form. The form can be downloaded by:

  • Download the form
  • Authorization to Participate in MyChildrensMercy's Patient Portal (Ages 0-17).
  • Authorization to Participate in MyChildrensMercy's Patient Portal (18+).
  • You can pick one up at Adele Hall Campus Health Information Management, 2401 Gillham Road Kansas City, MO 64108.
  • To have an email, fax or mail form sent to you, call (816) 234-355

Important notes:

If your age is over 18, To submit an application, you will need to attach a copy of a valid form of identification such as a driver's licence or school ID. You can give access to your parents by signing the Proxy Access section.
If your child is under 18 years old, you will need to attach a copy or legal documentation to the application. This document will state that you have been appointed guardian by the court. You may be granted permission by the patient to access their account.

Step 3

  • When the forms are processed, you will receive an email invitation to access the patient portal. Accept the invitation.

  • When you log in to the portal, you will be asked for your password change.
  • Your child's birth date and security questions will be required.
  • The account will be created using your information.
